This position is for a Care Coordinator/Case Manager who supports the mission of “Saving Lives, Instilling Hope and Restoring Relationships” in an integrated team with patients receiving counseling, case management and medically assisted treatment. **This position will support our Martinsville clinic 3 days/week, Roanoke and Danville 1 day/week**
The Care Coordinator (Case Manager)'s responsibilities include, but are not limited to the following:
- Conduct a thorough assessment of the patient’s needs, including arranging for specialized services, as needed
- Develop and implement the care coordination plan in collaboration with the patient, including but not limited to use of community resources, vocational training programs, and other needed services
- Advocate on behalf of the patient for needed services and benefits. Assist in obtaining reimbursement and benefits including, medical assistance, transportation, educations, etc.
- Provide education, modeling, and support in the areas of daily living, budgeting, money management, and medication management
- Engage patient in recovery-oriented, patient centered care in accordance with professional standards of practice
- Complete required Documentation in an appropriate and timely manner
- Ensuring compliance with HIPPA and CFR 42 part 2 regulations
- Collaboration with all members of the multidisciplinary team to enhance patient experience and continuity of care.
Requirements
Education:
- Bachelor’s degree in one of the following fields (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling)
OR
- Licensure by the Commonwealth of Virginia as a Registered Nurse (RN)
OR
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision as defined in 18VAC115-30-10 et seq.
Experience:
- One year of substance use related direct experience providing services to individuals with a diagnosis of substance use disorder.
OR
- A minimum of one year of clinical experience working with individual with cooccurring Diagnoses of substance use disorder and mental illness.
