Hospice Nurse Case Manager

As a Hospice Nurse Case Manager, your responsibilities include overseeing the comprehensive care of hospice patients, ensuring they receive comfort, support, and respect throughout their final stages. Effective teamwork and clear communication skills are crucial for this role, in addition to clinical expertise and a caring demeanor. $40 - $45 hourly

• Conduct comprehensive patient assessments, including monitoring vital signs and reviewing medical histories, to ensure a thorough understanding of their condition and needs.
• Develop individualized care plans that take into account medical evidence, cultural considerations, and ethical considerations, to provide personalized care to each patient.
• Safely administer medications and treatments as prescribed, following all necessary protocols and ensuring patient safety.
• Educate patients and their families on their health conditions, treatment plans, and preventive measures, to empower them to actively participate in their care.
• Collaborate with physicians and other healthcare professionals to ensure a coordinated and holistic approach to patient care, promoting optimal outcomes.
• Maintain accurate and up-to-date health records, including progress notes and care plans, in compliance with regulatory guidelines, to ensure continuity of care.
• Perform clinical duties such as IV therapy, wound care, and catheterization, with the utmost professionalism and adherence to best practices.
• Provide emotional and psychological support to patients and their families, particularly during challenging times, to promote comfort and well-being.
• Adhere to infection control protocols rigorously to prevent the transmission of diseases and maintain a safe environment for patients and staff.
• Stay informed about the latest medical advances and maintain necessary certifications, to continuously enhance knowledge and skills.
• Contribute to quality improvement initiatives by actively participating in audits and safety evaluations, to enhance the overall delivery of care.

• Must hold a valid RN license issued by the state of New Jersey.
• Current certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) are required.
• Previous experience in hospice care is preferred.
• Excellent communication, interpersonal, and teamwork skills are essential.
• Demonstrated commitment to delivering compassionate care.
• Strong attention to detail and ability to remain composed in stressful situations.
