Responsibilities :
Provide therapy sessions that address patient needs, facilitate self-disclosure, and promote insight into personal issues, using effective interventions and experiential techniquesWork closely with the Treatment Team to develop, review, and update personalized treatment plans with measurable goals and objectivesConduct psychosocial and bio-psychosocial assessments, gather diagnostic information, and present findings clearly to inform treatment planningAssist patients in preparing for post-discharge by developing a supportive discharge plan and connecting them with internal and external resourcesCommunicate with families, referral sources, and other service providers, ensuring accurate, timely, and secure documentation of patient progress in clinical recordsWorking hours : 8 : 00 AM - 5 : 00 PM
