Overall Function:
Assesses service needs and provides assistance in linking and accessing health and social service systems and interventions as identified through a life domain appraisal.
Essential Functions:
- Acts as the individual/family’s single point of contact for multiple health and social service system linkages, navigation and referral to address barriers to substance use treatment services and recovery.
- Links individuals/families to supports and recovery focused services.
- Identifies treatment and recovery barriers and ‘walks with the individual’ to navigate through the identified barriers.
- Monitors the individual/family’s participation in services and provides support to encourage participation.
- Assists in addressing social determinants of health which may be a barrier to treatment and recovery.
- Coordinates and maintains contacts and visits with referral sources and represents the program and agency at various training sessions, community education presentations and meetings.
- As necessary, participates in all appropriate staff meetings and maintains beneficial dialogue with all members of the community referral sources, as necessary, regarding assigned clients.
- Maintains current client case record activity and progress; prepares and submits all required program reports and documentation in an accurate and timely manner.
- May participate in the staff on-call after hours rotation process to assist clients, telephonically, with accessing higher emergent levels of treatment care.
- Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outline by federal and state laws, and agency policies and procedures.
Qualifications
- Associate degree in social work, chemical dependency, psychology, criminal justice or related human services field from an accredited college or university; OR any equivalent combination of experience and training.
- Has basic computer skills commensurate with the position and ability to learn a computer based electronic health records system.
- Demonstrates knowledge of substance use, mental health programs and available social services OR possesses the skills to learn such.
- Demonstrates ability to obtain vital information during an interview to make proper service recommendations.
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
- Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Fingerprint Clearances.
