The Accountabilities of Direct Support Professional in Community Participation
- Implements and monitors each person’s individualized plan to achieve specific outcomes, derived from individual’s preferences, needs and interests.
- Implements and maintain behavioral plans as needed.
- Communicates positively and effectively with all internal and external team members in developing assessments that determine desired outcomes, needs and preferences.
- Provides clear, detailed documentation in daily progress notes related to the individual’s outcomes, health, personal behavior, relationships, social interaction and any other pertinent information.
- Supports individuals to have the best possible health by teaching skills, providing supports and encouraging healthy lifestyles.
- Ensures that all individuals are actively participating and engaged to the fullest extent possible.
- Provides support to individuals to develop and facilitate meaningful daily/weekly/monthly Community Participation schedules.
- Supports individuals to learn to navigate the local community, including learning to use public transportation.
- Supports individuals to participate in community activities, organizations, groups associations or clubs to develop social networks and reciprocal relationships.
- Supports individuals to participate in desired activities related to the development of hobbies or leisure/cultural interests that provide purpose and responsibility.
- Supports individuals to explore and participate in volunteer opportunities.
- Supports individuals to develop skills and competencies necessary to pursue competitive integrated employment.
- Supports individuals in adult learning education, self-determination and self-advocacy opportunities.

The tools you need:
- High School Diploma / GED;
- Must be at least 18 years of age;
- Must have a valid PA/NJ Driver’s License and 1 year of driving experience
- Must be comfortable driving your own licensed and insured vehicles
