Job purpose
This position is responsible for assisting the clinical and admissions department. Case managers perform several tasks to achieve the best outcomes for the clients. These duties usually involve working with the clients directly.
Duties and responsibilities
· Orient new clients to unit and program.
· Contacting all referrals, SCA’s, and legal agencies upon admission.
· Maintain positive relationships with referral sources and outside providers.
· Ensure proper documentation is in the chart, including all necessary consents.
· Provide periodic progress updates to SCA, referrals, and/or legal agencies.
· Assist aftercare coordinator with MAT, Case Management and CRS referrals.
· Obtain PO approval for MAT referrals.
· Place clients on the phone and/or computer to apply for Medicaid services.
· Coordinate client transportation needs for off-site activities, such as St. Vincent DePaul and obtaining photo ID’s.
· Maintain communication with client’s family members and referral sources (SCA Case Managers, CRS, Probation, Social Workers).
· Assist clients with obtaining continuances on court hearings.
· Accompany and transport clients for court or other appointments as assigned.
· Send family packets.
· Maintain an organized system of communication with counselors.
Perform other duties as assigned
Qualifications
- H.S Diploma or equivalent (Required)
- Ability to communicate accurately and clearly both orally and in writing (Required)
- Pleasant telephone manner (Required)
- Customer service oriented (Required)
- Knowledge of computers; specifically, experience in Microsoft Office (Preferred)
- Superior organizational skills and accuracy (Required)
- Excellent problem solving (Required)
Job Type: Full-time
