SpiriTrust Lutheran is now hiring a PRN Resident Care Coordinator (LPN for Assisted Living) at The Village at Sprenkle Drive in York, PA.
Education/Training: Graduate of an accredited school of nursing with a current unencumbered LPN license, Current CPR/AED certification (or obtaining certification within 30 days of hire)
Skill(s): Knowledge of State and Federal regulations, including Assisted Living regulations and proficient in assessment and organizational skills.
Experience: A minimum of one-year experience in a related position normally required
General Responsibilities:
- Oversees internal and external health related services and administers appropriate health care of residents in accordance with the Assessment and Support Plan.
- Facilitates communication and documentation related to resident health care.
- Oversees and manages Resident Care Partners, supporting the development and maintenance of a strong health services program
- Provides guidance, support and encouragement to the care team with a focus on vision and purpose
- Facilitates communication among the care team, supporting information sharing and problem-solving
