INTRODUCTION : The Health Educator will be responsible for client and community contact in both Diabetes and Health Education, maintain and oversee weight room, and provide transports for clients. Health Educator will need to be energetic, highly motivated, friendly outgoing person to conduct physical activities in the prevention and management of diabetes for the community. Health Educator will be responsible in gathering and organizing data for SOS and SDPI reports for STH. The Health Educator will assist in the coordination of the Diabetes Grant (SDPI), provide leadership, assistance, support, and coordination of services for community members of the Spokane Indian Reservation in all areas of diabetes health education, wellness, and prevention activities. The Health Educator will additionally provide health education classes, food demonstrations, and healthy community activities reservation-wide that the community members can participate in, to receive an incentive. The Health Educator will work in both the Tribal Health and Special Diabetes programs with the Community Project Manager and will play a strategic role in both programs to promote overall health, wellness, fitness, and nutrition education.
MAJOR DUTIES AND RESPONSIBILITIES :
- Provides transportation to and from Diabetes related appointments, Home Visits as necessary, collecting health history for clients, maintaining accurate case files, and more.
- Responsible for gathering and organizing data for SOS and SDPI (Diabetes) reporting purposes).
- Provide individual and group Health Education training and classes, food demonstrations, and healthy community activities reservation-wide open to anyone, to receive an incentive.
- Assist in the coordination of the Diabetes Grant (SDPI). Provide leadership, assistance, support, and coordination of services for community members of the Spokane Indian Reservation in all areas of diabetes health education, wellness, and prevention activities.
- Plans, designs, implements, and assesses a systematic comprehensive diabetes program in collaboration with Tribal Health staff. Serves as a Diabetes Team Leader.
- Assist in community outreach : create fliers, pamphlets, promote on social media, visit local departments and programs, disperse inform to pertinent programs and providers in Spokane, WA.
- Coordinate wellness / health education activities for Ford, Wellpinit and West End sites, Senior Center, and tribal employment sites Work with the David C. Wynecoop Memorial Clinic & Tribal Health staff on an on-going basis in developing and implementing diabetes prevention programs including health fairs, diabetes screening, education activities and exercise programs.
- Knowledge of, or willingness to learn tribal, county, state and federal programs / opportunities which will benefit the health and wellness of the Spokane Tribal community.
- May assist other departments of HHS as needed per partnership of SDPI & fellow HHS Divisions.
- Responsible for Weight Room that is located inside the Wellpinit Boys and Girls Club including but not limited to : stock supplies, receive and process Weight Room Forms, Key Fobs, and maintain an organized system of users, vacuum floors, etc., clean equipment, monitor and create schedule, and other Weight Room duties as assigned.
- Ability to support and work closely with the Special Diabetes program and local I.H.S. on all projects and other duties as assigned for the Special Diabetes program.
- May occasionally be assigned after-hours or on a rotating schedule to assist with oversight to the Tribal Health Weight Room before, during, and after hours as needed.
- Assist program manager and admin assistant in maintaining case files and client records including health plans and determining eligibility for services / implementation of services.
- This position requires travel.
- Other job-related duties as assigned.
