RATE OF PAY : $21.00 - $23.10 / PER HOUR

INTRODUCTION : The Health Educator will be responsible for client and community contact in both Diabetes and Health Education, maintain and oversee weight room, and provide transports for clients. Health Educator will need to be energetic, highly motivated, friendly outgoing person to conduct physical activities in the prevention and management of diabetes for the community. Health Educator will be responsible in gathering and organizing data for SOS and SDPI reports for STH. The Health Educator will assist in the coordination of the Diabetes Grant (SDPI), provide leadership, assistance, support, and coordination of services for community members of the Spokane Indian Reservation in all areas of diabetes health education, wellness, and prevention activities. The Health Educator will additionally provide health education classes, food demonstrations, and healthy community activities reservation-wide that the community members can participate in, to receive an incentive. The Health Educator will work in both the Tribal Health and Special Diabetes programs with the Community Project Manager and will play a strategic role in both programs to promote overall health, wellness, fitness, and nutrition education.

MAJOR DUTIES AND RESPONSIBILITIES :

  • Provides transportation to and from Diabetes related appointments, Home Visits as necessary, collecting health history for clients, maintaining accurate case files, and more.
  • Responsible for gathering and organizing data for SOS and SDPI (Diabetes) reporting purposes).
  • Provide individual and group Health Education training and classes, food demonstrations, and healthy community activities reservation-wide open to anyone, to receive an incentive.
  • Assist in the coordination of the Diabetes Grant (SDPI). Provide leadership, assistance, support, and coordination of services for community members of the Spokane Indian Reservation in all areas of diabetes health education, wellness, and prevention activities.
  • Plans, designs, implements, and assesses a systematic comprehensive diabetes program in collaboration with Tribal Health staff. Serves as a Diabetes Team Leader.
  • Assist in community outreach : create fliers, pamphlets, promote on social media, visit local departments and programs, disperse inform to pertinent programs and providers in Spokane, WA.
  • Coordinate wellness / health education activities for Ford, Wellpinit and West End sites, Senior Center, and tribal employment sites Work with the David C. Wynecoop Memorial Clinic & Tribal Health staff on an on-going basis in developing and implementing diabetes prevention programs including health fairs, diabetes screening, education activities and exercise programs.
  • Knowledge of, or willingness to learn tribal, county, state and federal programs / opportunities which will benefit the health and wellness of the Spokane Tribal community.
  • May assist other departments of HHS as needed per partnership of SDPI & fellow HHS Divisions.
  • Responsible for Weight Room that is located inside the Wellpinit Boys and Girls Club including but not limited to : stock supplies, receive and process Weight Room Forms, Key Fobs, and maintain an organized system of users, vacuum floors, etc., clean equipment, monitor and create schedule, and other Weight Room duties as assigned.
  • Ability to support and work closely with the Special Diabetes program and local I.H.S. on all projects and other duties as assigned for the Special Diabetes program.
  • May occasionally be assigned after-hours or on a rotating schedule to assist with oversight to the Tribal Health Weight Room before, during, and after hours as needed.
  • Assist program manager and admin assistant in maintaining case files and client records including health plans and determining eligibility for services / implementation of services.
  • This position requires travel.
  • Other job-related duties as assigned.

KNOWLEDGE, SKILLS, & ABILITIES REQUIRED :

  • General knowledge of Native American health disparities including diabetes, blood pressure, sugar control, and more. Scheduling of various exercise programs in and around the Reservation to meet the needs of the program and the communities and to coordinate with other Diabetes program activities.
  • Ability to complete, record, and store fitness assessments of participants to ensure that each participant is physically capable to participate in physical activities.
  • Ability to conduct individual and group classes and activities.
  • Ability to work with a multi-disciplinary team of health care providers
  • Knowledge of the local Spokane Indian Reservation community and resources
  • Knowledge of and experience working within a Native American community
  • Ability to give oral presentations and prepare written reports.
  • Ability to collect, analyze and report data.
  • Ability to keep current and accurate records.
  • Ability to work with minimal supervision, collect, organize, and report data.
  • Knowledge of Indian Health Service and other health services.
  • Ability to travel and attend training seminars, continuing education programs, meetings and conferences.
  • Proficiency skills in Microsoft Office, Outlook, and other software systems
  • Experience working with all ages including youth, adults, and elders.
  • Ability to create an effective marketing campaign.
  • Must be physically fit and ability to participate in group or individual activities that require jumping, running, walking at a fast pace, as requested per on-going activities.
  • Able to meet the physical requirements of the position including but not limited to : able to lift up to 50 lbs, stand / sit / walk for extended periods of time, squat / kneel, climb stairs / ladders, reach overhead.
  • Ability to energize groups and individuals to implement new programs around policies and healthy activities
  • MAJOR DUTIES AND RESPONSIBILITES :

  • Follow developed protocols to ensure that exercise programs / activities are safe and effective for all participants including certain physical limitations persons with diabetes may have which would require special attention and individualized programs.
  • Fulfill role as a mandated reporter.
  • Provide direct care and support services to clients / community that are active clients of the Diabetes Program and / or community participants of Health Education Activities.
  • Coordinate and develop reservation wide services, outreach services, and assist clients in obtaining medical care when needed.
  • Assists in promoting cultural health information, and food sustenance, and be willing to expand knowledge on native foods, plants, and more. Assist STH in developing health goals for clients and the program.
  • MINIMUM QUALIFICATIONS

  • Minimum 6 months education and / or experience in a related or helping field (social services, case management, behavioral health, senior services, education, etc.).
  • Valid Washington State Driver's License, good driving record for insurance
  • Demonstrated previous Customer Service experience and skills
  • Must be able to always conduct themselves in a professional and ethical manner in their department and in the community.
  • Must be physically able to perform the job duties including leading group and individual exercise, lift, jump, bend, kneel, climb, etc.
  • Must be able to successfully pass all background and drug screenings
  • CERTIFICATIONS REQUIRED :

  • National Academy of Sports Medicine on Exercise Certification or be able to obtain certification within 6 months to 1 year of employment.
  • Ability to obtain CPR / First Aid / Blood borne pathogens within first 6 months of hire.
  • Ability to obtain Food Handlers certification within first 6 months of hire.
  • Ability to obtain Nicotine Dependence Certification, Basic Tobacco Intervention Skills Certification, Adult Mental Health First Aid within first year of hire (dependent upon training availability).
