Description
The certified nursing assistant provides direct care to the patients within the boundaries of training under the supervision of RN.
Check assignment; make rounds of assigned patients.
Reports any changes observed in condition or behavior of the patient.
Demonstrates knowledge of equipment and supplies used on the unit. Maintain a safe and clean environment. Report equipment that is out of order or in need of repair. Cleaning equipment.
Prepare the patients for meals, distribute food trays, assist or feed patients necessary and remove tray. Distribute fresh water and encourage fluid intake.
Provides morning care and evening care.
Correctly measure vital signs; takes and records weight, height, intake, and output measurements.
Reports any unusual actions or symptoms of patients.
Ask for assistance and give assistance as necessary.
Complete work assignment, leaving area clean, neat, and stocked.
Complete resident / patient assignments with all routine and ordered phases of care finished.
Recognizes and reports age-specific needs of patients.
Transports patients from various areas.
Assures confidentiality.
Assists in admissions, transfers, and / or discharge process.
Collects specimens and transports same.
Under supervision, performs general care activities utilizing infection control standards.
Handles and cares for patients' clothing and other personal property from admission through discharge.
Adjust own actions to compensation for behavior of the patient under the stress of illness.
Acts as focus of communication in the nursing station.
Receives requests from patients and families and refers requests to nursing staff.
Maintains adequate quantity of forms, materials, supplies, and equipment on the unit.
Maintains assignment boards, bulletin boards and other communication sources.
Maintains a clean, safe, and functional working environment.
Patient Record / Chart Activities :
Initiates, assembles, and maintains charts for all patients on the unit.
Initiates and prepares Dietary lists and other patient data as requested using CPSI system.
Assembles age-specific records.
Records and charts clinical data.
Initiates and maintains patient Kardex.
Files diagnostic and laboratory reports.
Maintains the patient chart.
Communicates all pertinent patient care information to the Head Nurse or Charge Nurse and other unit staff.
Completes daily staffing assignment.
Completes daily census sheet.
Cooperates with other team members and disciplines to promote maximum facility care of patient.
Develop the ability to anticipate needs and moods of all ages of patients.
Participates in staff meetings, continuing education programs, continuous quality improvement programs, and quality assurance. Attends mandatory in-services yearly, CPR, Fire, Safety, Disaster, and Infection Control. Complies with universal blood and body fluid precautions.
Adheres to Human Resources and departmental policies and procedures.
Follows HIPAA (Health Insurance Portability and Accountability Act) protocols.
Upholds Compliance Objectives to include obligation to report.
Is able to assess the developmental needs of patients form infant to geriatric and plan / adjust care, services, and instruction appropriately within the scope of his / her responsibilities.
Exhibits and promotes a positive attitude and outlook in the workplace and in the community. Demonstrates behavior that is conducive to the professional operation of the organization.
Participates in and is an advocate of departmental and interdepartmental communication within facility to facilitate improved customer service and patient outcomes.
Reports to work when scheduled and demonstrates flexibility of personal time when requested to come in on a scheduled day off.
Adheres to facility and departmental safety policies. Notifies supervisor of any safety concerns / hazards to staff and / or patients / patients.
Knowledgeable regarding Hospital / Nursing emergency procedures.
Successfully completes required annual job-specific competency testing.
Work time is used productively, equipment and supplies are taken care of properly (cost conscience use of supplies, disposable / non-disposable / recycled), breaks and lunches are within the time allotted.
Assist patients to and from bathroom, offer bedpans, and keep incontinent patients clean at all times. Transfer on and off commodes.
Groom patients as attractively as possible and encourage attendance at recreational therapy, church services, and programs as scheduled.
Maintain appropriate activity level of patients, including range of motion, exercise, walking, up in chairs, etc. as ordered. Also, reposition bed patients routinely, lifting and turning with proper body mechanics and applying and utilizing special equipment as necessary.
Demonstrates proper lifting and turning of patients using the proper body mechanics and applying and utilizing special equipment as necessary such as gait belts and lifting devices.
Additional duties as assigned by Supervisor.
Requirements
Qualifications
- Completion of the standard Nursing Aide course and registered with State Registry.
- Basic computer skills with ability to learn EPIC system of order entry and flow chart documentation.
- Ability to work with all ages of patients.
- Pleasant, cheerful, tactful, courteous, and calm with patients and visitors.
- Ability to cooperate with other personnel and work under direction.
- Ability to read, write, and comprehend medical terminology.
- BLS Certified.
