What are the responsibilities and job description for the NUTRITIONIST position at Sprain Brook Manor Rehab?
The Nutritionist will perform individual counseling of patients referred by the medical personnel. Performs job duties in compliance with systems and job specific standards. Essential Job Duties and Responsibilities:
- Provides nutrition counseling to patients referred by clinicians
- Performs initial nutrition history on each patient
- Develops and implements a care plan based on the assessment of nutritional and health needs
- Provides instructions on a normal or special diet
- Schedules follow-up appointments and evaluates the reaching of goals
- Provides nutrition counseling for diabetic patients as part of diabetic care plan
- Provides nutritional education materials to patients to support counseling efforts
- Makes referrals to other health care providers and community services as appropriate
- Counsels individuals and families on nutrition and diet based on the particular needs of the individual or family as determined by their nutritional and health risk
- Attends staff meetings and in-service training
- Provides consultation into the development of program policies and procedures
- Provides direction and executes health education programs as needed
- Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served Minimum Education Requirement : Bachelor’s Degree Related Work Experience : 2-4 years Required Licensure: RD/ RDN or CDN with a valid CPR card Specialized Skills & Knowledge:
- Ability to communicate in English, both orally and in writing
- Knowledge of nutrition principles as well as knowledge of nutrition during pregnancy and lactation, as well as infant and child feeding practices
- Competence in nutrition assessment counseling
Salary : $40 - $48
