What are the responsibilities and job description for the Full Time Weekend Hospice RN - North position at Sprenger Wellspring Silver Maple?
- BASIC PURPOSE
The on-call RN is a key component of the hospice team that maintains the hospice service coverage evenings and weekends by answering and responding to all patient/family telephone call, referrals, and requests for assistance. As an RN the position completes admissions for hospice care, performs follow-up patient care visits, provides visits on patients with emergency needs, and pronounces death. The RN is responsible to communicate significant findings, problems, patients needs, reports changes in condition or environment to the physician, Assistant Clinical Director and other members of the hospice care team and obtains physician orders as required. This important position must maintain flow of communication between the office staff, field staff and other departments, for coordination of activities and services.
Participates as a member of the interdisciplinary team in hospice interdisciplinary team meeting. Functions in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.

Nursing Process
As a member of the IDT, the Registered Nurse (RN) utilizes the nursing process to contribute to each patients total plan of care.

Assessing and Planning
- Seek and obtain sufficient information necessary to function effectively in the Hospice setting where the patient/family/caregiver is receiving care.
- Conduct initial assessment, including physical, emotional, and psychosocial aspects, of patient/family/caregiver in a thorough, accurate, compassionate, and timely fashion. Admitting paperwork and patient care plans must be submitted on the same day of admission. Assures that all paperwork is completed in full at time of submission for timely data entry, IDT and plan of care information.
- Submits re-certification paperwork (new Medication Profile, updated plan of care, updated Hospice Aide plan of care) within 24 to 48 hours.
- Reports significant patient status changes and needs due to changes to other disciplines and appropriate agency staff, Clinical manager and patients physicians. Submits change orders same day of occurrence.
- Utilize the nursing body of knowledge to provide leadership and guidance to the IDT in formulating and individualizing the interdisciplinary plan of care to include problem identification, interventions, and goals to address palliative comfort measures for the terminally ill patient, family and caregiver.
- Contribute to identifying both active and potential needs of the patient and his/her family, which may affect the plan of care.
- Consistently collaborate with the physician to plan and implement the medical and nursing plan of care.
