Qualifications: 2 years of experience in long term care, licensed social worker or bachelor’s degree preferred, must possess excellent communication and organizational skills
Essential Job Functions:
- Develop one-to-one relationships with residents and families.
- Establish relationships with area hospice agencies.
- Act as a liaison with social, health and community agencies.
- Coordinate with Activity Department to meet socialization needs of the residents.
- Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).
- Assist in admissions and discharge process as needed.
