What are the responsibilities and job description for the Social Service Worker position at Spring Creek Rehab?
Employee is responsible for developing, planning, implementing and evaluating a comprehensive social service programs and services in accordance with State and Federal Laws and Regulations. To identify and provide for residents’ social, emotional, spiritual, cultural, and psychological needs. To assist in the discharge planning upon admission and throughout stay according to Facility Policy. To provide resident’s service coordination within the facility and externally. Provide resident, staff and family counseling.
Qualifications, Knowledge, and Skills:
o Licensed Social Worker according to State and Federal Regulations in which the facility operates
o Bachelors or Masters Degree in related field, preferred
o Must be able to communicate effectively
o Working knowledge of nursing home regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid
o Must meet all Federal, State and Local Health Regulations
o Must be able to communicate via English, written and orally
o Must be capable of performing the Essential Functions of this job with or without reasonable accommodation
o Using logic and reasoning to identify strengths and weaknesses of alternate solutions, conclusions or approaches to problems
o Knowledge of principles and processes for providing customer services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
Social Service professional will maintain confidentiality and security of all PHI, (Personal Healthcare Information), defined in HIPAA (Health Insurance Portability and Accountability Act) unless the resident or employee consents or authorizes the use or disclosure of PHI or that which is permitted by the HIPAA Privacy Standard. This includes healthcare information, oral or written, which is individual identifiable information relating to a resident or employee’s healthcare needs or payment for healthcare.
