- Position Description : SUMMARY OF POSITIONProvide case management services to older clients and families in accordance with the standards of the social work profession and requirements of the funding source to those 60 years and older residing in Spring Creek Towers allowing them to successfully age in place. Provide services to large number of Russian-speaking clients of the SCSP NORC but not exclusively.DUTIES & RESPONSIBILITIES\tInterview clients, conduct NORC Assessment, develop NORC plan of care, monitor implementation and adjust plan as required by funding source and adhering to standards of the social work profession;
tProvide case management and case assistance services in accordance with standards of the social work profession and grant requirements and guidelines
tProvide case management services as determined by needs of program and under supervision of the Program Director.
tWork closely with NORC Nurse and provide documented evidence of interdisciplinary care planning required
tConduct Health Indicator surveys as indicated by Program Director and as required by funding source. Use results of surveys to guide practice as indicated by Program Director and as required by funding source.
tRefer clients to community resources as indicated by assessment of needs and NORC Care Plan
tWork with the Program Director to develop procedures, schedule workflow, assignments and ongoing programs assuring that these comply with all contract, grant, nonprofit guidelines, and social work professional standards
tWork with the Program Director to develop programs based on documented needs of the community of seniors
tWork with Program Director to investigate and develop resources to meet specialized needs of groups in community
tMaintain accurate records and provide timely statistical and activity reports. Assure that information is up-to-date and assessed on an annual basis. Client confidentiality agreements and Release of Information forms renewed annually.
tAssure contracted levels of units for responsible areas are achieved each month.
tComply with all present and future requirements of contracts and grants
tParticipate in case conferences and supervision sessions; work as part of interdisciplinary team to provide coordinated services to seniors; present cases during case conference meetings and adjust care plan as indicated
tActive participation in supervision sessions with supervisor
tParticipation in continuing education required
tRemain current in general office technology
tPerform all other duties as assigned.

tGraduate of an Accredited College or University with a Bachelor of Social Work (BSW) degree preferred or BA in Human Services and experience providing services to Older Adults. (Russian Speaking preferred but not required). AA, AS Degree required with 3 years of Case Management experience.

WORK ENVIRONMENT
tThis job operates in a professional office environment, periodically at nearby community center and other locations within the Spring Creek Towers complex. Occasional meetings in other boroughs of NYC.
tCase load is heavy, work is intense, often crisis oriented, and unpredictable; many clients and families have multiple and complicated situations.
tAssessment and services provided to homebound seniors in their homes
tThis role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines, etc.
