Position Summary:
The incumbent will serve as an Outpatient/Inpatient Social Worker/Registered Nurse (RN) Case Manager for TCRHCC and Sacred Peaks Healthcare Center (SPHC). The primary responsibility is the direct supervision and supportive contact for a designated group of high-risk patients. The Social Worker/RN Case Manager will design and manage a continuum of care, focusing on empowering clients to achieve measurable outcomes and self-sufficiency. This includes conducting assessments, developing service plans, securing necessary resources, monitoring patient progress, and addressing emerging needs. The role will also involve linking clients to community social services, healthcare providers, substance abuse, and mental health services to meet specific treatment goals.
The Social Worker/RN Case Manager is responsible for performing utilization reviews of patient cases within both inpatient and outpatient services, ensuring the medical necessity of referrals to specialty providers for services not typically offered within TCRHCC. The incumbent will collaborate with a multidisciplinary team to optimize the use of healthcare resources, focusing on the patient as a whole to ensure a seamless continuum of care. The goal is to provide quality care, reduce care fragmentation, enhance patient self-care and quality of life, and control costs. The incumbent will work closely with providers, nurses, social workers, physicians, the PRC department, and other healthcare staff at TCRHCC to implement appropriate resources for utilization goals as defined by the hospital and third-party payors. The Social Worker/RN Case Manager will report to the Director of Case Management/Care Coordination.
Education:
- MSW Social Worker: Master’s Degree in Social Work (MSW)
License/Certification:
- Must maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment.
- MSW Social Worker: Must hold a valid, current, and unrestricted Professional License to practice as an LMSW or LCSW in any state of the United States, the Commonwealth of Puerto Rico, or a U.S. territory.
Experience:
- A minimum of three (3) years of experience in a clinical care setting.
- Experience and knowledge of discharge planning, case management, or utilization review processes.
- Demonstrated knowledge of electronic health record systems.
