Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. This individual will also assist the MDS coordinator in accurately and timely submitting MDS assessments.
Duties:
- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Provide counseling and support services to clients, including crisis intervention
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to coordinate services and resources
- Assist clients in accessing community resources, such as childcare, housing assistance, Medicare, and workforce development programs
- Monitor clients' progress and make necessary adjustments to care plans as needed
- Advocate for clients' rights and ensure they receive the appropriate services and support
- Maintain accurate and up-to-date case notes and documentation
Experience:
- Previous experience in case management or a similar role preferred
- Knowledge of child welfare systems and behavioral health services
- Strong understanding of crisis intervention techniques
- Excellent communication skills, both written and verbal
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
