Part-time Home Health Aide
Full job description
We are looking for a compassionate Home Health Aide to help in daily living and personal care services in patients’ homes in accordance with an established care plan. Must be an active CNA on the Illinois Healthcare Worker's Registry.
Responsibilities
- Provide health care services in patients residences
- Perform domestic and household tasks
- Assist with client’s personal care activities
- Monitor patients (vital signs, temperature, respiration, etc.) and report on their condition to the case manager (RN/PT)
- Maintain patients care records and document provided services through AxxessCare
- Instruct and counsel patients and families on diet and exercise
- Provide companionship and basic emotional or psychological support
- Provide other duties as per the Plan of Care developed by the patient, RN/PT, and MD
Skills
- Working experience as CNA or home health aide, required
- Ability to monitor vital signs
- Familiarity with basic nutrition and personal hygiene standards
- Caring and compassionate personality
- Current CPR certificate
- First Aid training
- Valid driver’s license and car insurance, reliable transportation
Qualifications
- High School Graduate or equivalent required.
- Completed a State-certified CNA or other approved training program (484.36a)
- Complete and pass the Spring Meadows Home Health Care performance or competency skills test.
- Cannot be unemployed for a period of 24 month after passing the State approved training and competency evaluation program
- Ability to read, write, and carry out directions.
- Good verbal & written communication.
Job Type: Part-time
Pay: $17.00 - $19.00 per hour
Expected hours: 10 – 30 per week
