SUMMARY
Under the direct supervision of the Senior Director of Client Services, the Case Manager will provide crisis services and short-term service coordination and follow-up to victims of domestic violence. This position is responsible to complete intakes with victims, providing case management, crisis counseling and shelter services where applicable. The goal of the intervention is to assist all domestic violence victims to increase their safety as well as appropriate resources. The goal of the intervention is to assist victims increase their safety, to assist with stabilization of the family, and assist them to utilize appropriate resources.
PRIMARY DUTIES AND RESPONSIBILITIES
· Work is not limited to shelter clients only, will serve all DV referred clients, whether it is a primary or secondary victim
· Provide crisis counseling, information and referrals regarding resources in the county, legal and financial resources to women in acute crisis following a domestic violence incident.
· Create a comfortable and safe stay for victims and children
· When applicable, respond to requests for entry into the shelter; these requests occur from the 24hour domestic violence hot line and the Lethality Line. Staff will assess for appropriateness of placement and ensure safe placement of family in danger.
· In collaboration with other case managers and staff who work within the Domestic Violence program, respond to the needs of victims who contact the 24-hour hotline.
· In collaboration with other case managers who work within the Domestic Violence program, respond to the needs of victims who contact SCS through Lethality Assessment Protocol (LAP) phone and may be responsible for follow up LAP phone contact.
· Conduct a DV case management assessment on all victims who enter and provide all victims with an opportunity to create a service plan with goals that meet the victim’s current needs.
· Create personalized safety plan for each victim and family and update as often as necessary.
· Educate victims regarding issues related to domestic violence, healthy relationship and personal safety
· Conduct peer review audits
· Attend court hearings as needed to provide client support and resources for the court system
· To ensure safety, respond to the call in phone to check in with victims who are in the community in the absence of the Support Staff.
· Maintain and provide a packet regarding domestic violence information, resources, and services to be provided to victims referred to the program.
· Participate in ongoing Outreach activities within the Case Management program.
· Maintain collaborative working relationships with outside agencies.
· When applicable, will transport clients to appointments in agency vehicle
· Provide service coordination and short-term follow up for victims of domestic violence.
· Work one late day/evening in order to participate in the planning and implementation of group activities as assigned or meet with assigned clients
· Participate in scheduled supervisory sessions with the Coordinator to provide feedback regarding direct services, program and other issues which may impact the program.
· Other duties as assigned.
GENERAL DUTIES
· Active participation in team meetings to assist with ensuring the clients receive appropriate and timely services and to provide input to enhance service delivery for program participants.
· Keep notes on victims updated to the day in the confidential electronic files system used by the agency.
· Maintain statistics regarding service delivery and other services provided to victims of domestic violence as necessary.
· Maintain awareness of community resources.
· Manage tasks assigned.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.
EDUCATION and/or EXPERIENCE
· Bachelor’s degree in Social Work, Human Services or related field.
· A minimum of one-year experience providing crisis services to families.
· Knowledge of and ability to work with persons of diverse backgrounds.
· Knowledge of crisis counseling skills.
· Knowledge of dynamics of domestic violence.
· Current Driver’s License
· Safe Driving Record
LANGUAGE SKILLS
Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients, collaborative agencies or employees of the Agency.
MATHEMATICAL SKILLS
Basic math skills of addition, subtraction, multiplication and division are required.

· Will need to travel to meetings, hotels, etc. to meet with clients and attend meetings. This must be done in a timely and efficient manner.
