What are the responsibilities and job description for the Psychiatric Rehabilitation Program (PRP) Direct Care Counselors position at Springboard Community Services?
Springboard Community Services (SCS) is a CARF Accredited Outpatient Mental Health Clinic with offices in Baltimore City, Carroll, Harford and Howard Counties. We are looking to expand our services with the provision of a Psychiatric Rehabilitation Programs serving both minors and adults. Under the direct supervision of the Psychiatric Rehabilitation Program (PRP) Director, the PRP Direct Care Counselors will serve as an integral member of our team by providing goal oriented individualized rehabilitative services that enable clients to develop skills that enhance the quality of their lives. Rehabilitative services are developed in partnership with the client, their support system including parents/guardians (if applicable) and mental health professionals in key life domains such as self-care skills, social skills, independent living skills, occupational and/or educational goals, spiritual and leisure activities to facilitate community integration.
Under our fee-for-service model PRP Counselors will be paid monthly with the following rates: $25.00 per billable onsite or offsite client encounters and $35.00 for each combination of onsite/offsite billable client encounter. Counselors will be paid an hourly rate of $20.00 for 60 hours of required training; training must be completed before direct client services can be provided. Satisfactory completion of a minimum of 40 hours paid training is a condition of employment. All training hours are expected to be completed within 2 weeks of hire.
PRIMARY DUTIES AND RESPONSIBILITIES
· Provide direct services to clients in a variety of settings such as their home, community and/or office-based settings a minimum of 4-6 times monthly (may increase or decrease depending upon client’s needs and outlined goals).
· Maintain accurate and up-to-date documentation of all activities and/or interventions in the client's electronic health record and provide feedback/reports to the appropriate professional staff.
· Assist in the development or implementation of the clients Individual Rehabilitation Plan (IRP).
· Plan, organize and conduct rehabilitation assessment, evaluation, and intervention for a client on caseload, including orienting as well as ensuring the client's family and/or legal guardian when permitted in participating and developing the client's individual rehabilitation plan.
· Model and assist in developing coping skills and improving effective communication skills.
· Teach clients independent living skills, including identifying and addressing gaps in service provision as well as teaching client community integration skills.
· Conduct Rehabilitation groups and activities to address rehabilitation and support service needs.
· Maintain required and consistent documentation for each client, including contact/progress notes, monthly summaries, and quarterly reviews.
· Assist with development and implementation of individualized rehabilitation plans for assigned clients to meet individual needs, interests, abilities, and share information with clients on how to access community resources to meet their individual needs
· Collaborate monthly with the therapist of each client on your caseload and document this collaboration in the monthly summary.
· Attend mandatory monthly staff meetings.
· Evaluate, monitor, and review individualized rehabilitation plans and assess individual progress in the treatment and make recommendations for modification, changes, or termination of the treatment.
· Facilitate transition, discharge, termination, admission, and clinical supervision as scheduled to promote consumer services and follow-up care.
· Other duties as assigned.
GENERAL DUTIES
- Provide identified PRP activities and support services in accordance with the clients Individual Rehabilitation Plan (IRP).
- May act as liaison with appropriate and identified stakeholders in clients’ self-advocacy and self-identified goals.
- May act as a consultant to outside providers, school personnel, and medical staff on the individual needs of the client's caseload.
- Maintain records as required by established laws, policies, and administrative regulations including COMAR regulations.
- Participate in ongoing training and supervision as required.
- Manage additional tasks assigned.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities
EDUCATION and/or EXPERIENCE
High School or GED is required; Associates or Bachelor’s Degree in Human Services related field is preferred, ideal candidate will have at least one year of human services experience. Demonstrate excellent verbal, interpersonal and written communication skills. Ability to manage time effectively, patience, and willingness to engage in ongoing professional development as needed.
LANGUAGE SKILLS
Ability to write routine reports and correspondence. Ability to speak effectively before groups of clients, collaborative agencies or employees of the Agency.
MATHEMATICAL SKILLS
Basic math skills of addition, subtraction, multiplication and division are required.

· The Direct Care Counselors will work in the community in several settings such as client’s homes, community based organizations or other office based setting to ensure we are meeting the needs of the client’s.
· The Direct Care Counselors must be able to transport her/himself and clients to other locations as necessary in a timely manner.
