Position Summary :
To work as a team in managing the operation and compliance of the Day Habilitation program by providing training, coaching and supervision to Shared Day Habilitation Assistants with the guidance of the Program Coordinator. To ensure that all the staff are well trained in order to maintain a daily schedule of program activities that engage participants in choice making, community involvement, and onsite activities that assist the individuals with a structured and meaningful day.
Primary Duties and Responsibilities :
- Be a good role model / advocate for the people you support in all situations and settings
- Be punctual and dependable; Regular staff attendance is vital for the people we support
- In partnership with the people you support, observe, discuss, and help assess their strengths and opportunities to help develop their personal goals and plans
- Support and encourage people as they make choices about the goals they want to pursue, the activities they wish to participate in, and how they want to live their life
- Follow all written plans, learning materials, and goals
- Collect required data
- Complete required billing documentation for supports and services provided
- Obtain and maintain all applicable training, certifications, and knowledge of policies and procedures
- Ensure safe and clean environments, including grounds and vehicles, for the people we support
- Attend team meetings and 1 : 1 meeting with supervisor as assigned
- Assist the Program Coordinator in the development of a meaningful program (Day).
- Create relationships in the community by facilitating community activities.
- Maintain professional communication with families, other Springbrook Depts. and outside agencies.
- Give direction, have good communication skills, provide leadership to staff, and be able to perform the task of the Program Coordinator in their absence.
- Demonstrates good role modeling while working with the individuals on daily living skills, leisure and recreational activities.
- Assure the safety and well-being of each individual while promoting independence and respecting each individual's choices, needs, rights and responsibilities.
- Performs other related duties as assigned by the Program Coordinator.
- All other duties as assigned
