Job Summary
The Client Referral Specialist is a vital position focused on connecting individuals with developmental disabilities to home maker/personal care services. This role is responsible for referring individuals who need support to our services. The ideal candidate should have personal or community connections to individuals with developmental disabilities, ensuring they are able to make the necessary referrals. This position plays an essential part in improving the lives of those who require care services, and the focus will be entirely on making these referrals.

Duties

• Identify and refer individuals with developmental disabilities in need of home maker/personal care services.
• Maintain an active network of community connections, especially those with loved ones or individuals with developmental disabilities who require services.
• Ensure the timely referral of clients to appropriate care services.
• Collaborate with local organizations, schools, and community groups to identify and reach individuals in need of services.
• Document and track all referrals to ensure proper follow-up and care coordination.
• Provide potential clients with information on our services and answer questions regarding the referral process.
• Maintain up-to-date records of referrals and the status of clients in need of care services.
• Stay informed of community resources and opportunities to better serve individuals with developmental disabilities.
• Serve as the main point of contact for clients, families, and other community organizations in need of services.

Skills

• Strong community connections with individuals who have developmental disabilities or families with loved ones in need of services.
• Experience or familiarity with developmental disabilities and related care services.
• Excellent communication and interpersonal skills for working with clients, families, and community organizations.
• Ability to build and maintain relationships with local service providers and support networks.
• Strong organizational skills and attention to detail in managing referrals and documentation.
• Ability to engage with clients compassionately and efficiently, ensuring they have the resources they need.
• Knowledge of local resources and support services for individuals with developmental disabilities is a plus.
• Ability to work independently and as part of a team to meet the needs of the community.

Job Types: Full-time, Part-time

Pay: $17.00 - $19.00 per hour
