Job Relationships
This position reports to the Clinical Operations Manager.
Key Responsibilities
- Evaluate the psychosocial status of patients related to their illness and environment, and communicate findings to the registered nurse.
- Conduct thorough social evaluations and develop intervention plans based on evaluation results.
- Maintain accurate clinical records for all patients referred to social work services.
- Provide essential information and referral services to organization patients and families / caregivers regarding practical and environmental needs.
- Offer guidance to patients or families / caregivers and community agencies.
- Serve as a liaison between patients or families / caregivers and community agencies.
- Build collaborative relationships with organization personnel to support patient care.
- Negotiate contracts with public and private agencies as resources for patient and organization personnel.
- Participate in developing comprehensive patient care plans and case conferences as needed.
- Assist physicians and other team members in understanding significant social and emotional factors related to health issues.
- Collaborate in discharge planning.
- Pursue other duties as delegated by the Clinical Manager / Director.
- Comply with Springfield Clinic's incident reporting policy and procedures.
- Adhere to OSHA guidelines and Springfield Clinic training requirements.
- Foster excellent customer service and adhere to Springfield Clinic's Code of Conduct and Ethics Standards.
- Perform other job-related tasks as assigned.
