Job Summary
The Home Health Physical Therapist Assistant will administer comprehensive physical therapy modalities / regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.
Principal Responsibilities
- Follow plan of care developed by Physical Therapist using a variety of treatment techniques.
- Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination.
- Consult and practice with other health professionals involved in patient care.
- Provide quality, personalized, evidence-based care and proven interventions specific to patient needs.
- Encouraging and help motivate the patient during their recovery process.
- Completes required documentation to complete patients visits and episodes as required by federal, state and payor guidelines.
- Instructs patients and family / significant others in home exercise programs to facilitate function.

Working Environment
This job operates in a variety of settings including but not limited to professional office environment patients homes and car.
Routinely uses standard office equipment such as computers phones photocopiers filing cabinets and fax machines.
Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
Possible exposure to blood bodily fluids and infectious diseases.
