Job Description
The Home Health Physical Therapist evaluates, plans, and administers comprehensive physical therapy modalities / regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.
Principal Responsibilities
- Provides initial and ongoing assessment to determine level of function, and any other assessments as required by federal guidelines
- Establishes an in-home treatment plan in coordination with the physician and other care team members.
- Utilizes various evaluation and treatment techniques based on both physical and psychosocial levels of patient function.
- Completes required documentation to complete the patients visits and episodes as required by federal, state and payor guidelines.
- Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
- Provides quality, personalized and evidence-based care and proven interventions.
- Supervises therapy assistants and other home health staff as appropriate.
- Motivates patients during treatment in order to help them function optimally.
- Reduces the need for medications and provides alternatives to surgery.
- Evaluates effects and monitor and communicate progress.
- Consults with physicians regarding change in treatment.
- Writes reports to physicians regarding patient's progress.
- Instructs patients and family / significant others in home programs and activities of daily living.
