Unit Specific Information General Emergency Department-Combined Adult / Pedi experience required Additional Details A legible copy of a valid / current US Driver’s license or US ID card Badge photo required within a week of signing. This is due no later than Monday before start. Photo must be in color from mid-shoulders up. It must be taken against a plain background, preferably dark blue or dark gray with appropriate clothing and no filters. Not needed for any candidate extending from Focus One. Candidate must review and sign within 7 days of signing contract. Required if candidate is requesting a Religious Exemption from flu vaccine. Once completed, CL must send to PM for approval.
