Essential Functions of Social Services Assistant :
Assure that social service / discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levelsDevelop specific, measurable, realistic plans and objectives, which enhance social service dailyPerform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their familyPerform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical / health care errorsFormulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning. Include family in teaching as appropriate.Demonstrate knowledge of clinical social work and discharge planning. Interpret and promote residents’ rights and the “Residents’ Bill of Rights”Interact professionally with resident and family and involve resident and family in the formation of the plan of care.Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.Coordinate and direct Social Service / Discharge Planning Department to ensure resident needs are met and center policy is followed.Provide appropriate resources and support to Social Service / Discharge Planning Staff / Clinical Staff.Be flexible, organized, and function under stressful situations.Maintain a good rapport and working relationship with medical staff, departments, and employees.Manage and operate equipment safely and correctly.Treat patients and their families with respect and dignity.Support and maintain a culture of safety and quality.Prepares social histories for use by professional center staff and participates in resident care conferencesParticipates in the development and reassessment, as needed of each resident’s social service and interdisciplinary care planDemonstrates the ability to perform a psychosocial assessment on all residents and reassessments, including adult and geriatric residentsHandles problems of adjustment to the long-term care center and maintains frequent contact with residents and their familiesServes as an advocate for residents and their familiesEstablishes and maintains working relations with community health, welfare and volunteer groups to promote cooperative assistance in meeting the social welfare needs of residents and familiesQualifications of Social Services Assistant :
Bachelor’s degree in social work is preferredCurrent State Clinical Social Work Licensure is requiredShort-term rehab experience is preferred2 years of experience in social services background or a behavioral setting is preferredCandidates must maintain a working email address and phone number for employer communication.Ability to work cooperatively as a member of a teamAbility to communicate effectively with residents and their family members, and at all levels of the organizationAbility to react decisively and quickly in emergency situationsAbility to maintain confidentialityAbility to read, write and speak the English languageKnowledge of training techniques for clinical staffGood Ethical PracticeRelationship managementBasic computer knowledgeStrong written and verbal skillsEEO Statement :
