We’re currently seeking a qualified and energetic RN Home Care Coordinator - (20-24 hrs. per week) to join our well-established dynamic Home Care Agency team and assist in offering Springmoor Nonmedical Home Care Services in the Wake County area. Prefer experience in Home Care or Geriatrics. Potential to expand to full-time in the future.
Primarily weekday hours with some on call by phone required.
GENERAL RESPONSIBILITIES :
Acts as contact person / agency representative for clients seeking outside of Springmoor Non-Medical Home Care Services.
Performs a comprehensive client assessment.
Formulates a plan of care in conjunction with the client that is adequate to satisfy their needs.
Assigns appropriate staff to fulfill service needs as outlined in plan of care.
Assists in providing office-hours and on-call service supervision of caregivers as needed.
ESSENTIAL JOB FUNCTIONS :
Conducts the initial, comprehensive assessment in the client’s home prior to the development of the plan of care.
Formulates plan of care and revises as needed utilizing home care software system.
Conducts staff supervisory visits in the home every 90 days and annually; completes required documentation of visits.
Ensures clients receive the proper level and quality of care.
Responds to client and staff inquiries and infection control issues, facilitates referral to appropriate level of care, and notifies emergency contacts as needed.
Ensures appropriate staffing patterns for service delivery.
Participates in interviewing, hiring, and supervision of Home Care staff.
Assists with 90-day probationary and annual employee evaluations.
Assists with agency audits and annual comprehensive agency evaluation.
Assists in providing office-hours and on-call service / caregiver supervision.
Ensures completion and maintenance of client record including intake, care management, and discharge.
Oversees staffing and scheduling of caregivers.
Assists with staff in-service education.
Attends any mandatory or annual in-service training as specified by Springmoor Home Care policies.
QUALIFICATIONS :
Must be a Registered Nurse, licensed to practice in North Carolina.
A minimum of three years’ experience in geriatrics is preferred.
Must possess leadership ability and the willingness to cooperate with Supervisors, other Departments Heads, and personnel.
Must have the ability to plan, deliver and direct client care.
Must be proficient with computers, Microsoft Office Products
LICENSE OR CERTIFICATION :
Current, valid North Carolina Registered Nursing License
