We are seeking an experienced Resident Services Coordinator to join our team. The successful candidate will be responsible for developing and implementing case management services, facilitating family involvement, and promoting interactive problem-solving with staff. They will also assist with move-in and orientation of new residents, monitor and maintain records, and participate in resident-centered activities programs.
Responsibilities
• Develop and implement case management services to all residents, ensuring their physical, emotional, and mental wellbeing is supported.
• Facilitate family involvement through regular communication, events, and activities, promoting a sense of community and connection among residents and their loved ones.
• Assist with move-in and orientation of new residents, providing a smooth transition and ensuring they feel welcome and supported.
• Monitor and maintain accurate records reflecting residents' wellbeing, documenting incidents and progress as needed.
• Participate in and promote interactive problem-solving with staff, fostering a collaborative environment that prioritizes resident needs and satisfaction.
• Assist residents and their families with move-out procedures, if necessary, ensuring a seamless transition and minimizing stress.
• Develop, implement, and coordinate resident-centered activities programs, including intergenerational initiatives that bring residents, families, and the community together.
• Coordinate volunteer programs with residents and the community, promoting socialization, engagement, and a sense of purpose.
• Promote positive public relations through media outreach and special events, showcasing Springpoint Living's commitment to excellence and resident-focused care.
• Develop and maintain written policies and procedures, ensuring compliance with regulatory requirements and best practices.
Required Skills and Qualifications
• Bachelor's degree in social work, gerontology, or a related field.
• Minimum 2 years' experience in social work with seniors and non-elderly disabled individuals.
