What are the responsibilities and job description for the Housing Care Coordinator position at Springwell?
Springwell is seeking an enthusiastic and compassionate Care Coordinator to join our dynamic Housing team! This program brings care coordination into public and private housing sites for older adults and people with disabilities. Ideal candidates will have a strong desire to work with this population, excellent organizational skills, and the ability to manage constant change with a positive attitude.
DESCRIPTION:
- Provide regularly scheduled office hours in assigned housing sites to provide consultation and assistance for all residents of the building.
- Offer resource information and programming to residents.
- Conduct comprehensive in-home assessments with older adults.
- Help determine needs and eligibility for government subsidized services.
- Create care plans and coordinates services.
- Communicate and collaborate with housing management staff and community professionals.
QUALIFICATIONS:
- Bachelor's Degree required. Degree in Social Work, human services, nursing, psychology, sociology or related field preferred. Candidates with a degree in another discipline must demonstrate experience and/or strong interest in the field of human services via previous employment, internship, volunteer activity and/or additional studies.
- One year experience in human services field preferred.
- Experience with managing a variety of mental health issues preferred.
- Working with computers, entering data and running reports.
- A reliable car, current driver's license and safe driving record required.
