Job Description
We are urgently hiring RN Case Managers for our Home Health team in Clearlake, Simi Valley, and Ukiah, CA. As an RN Case Manager, you will play a key role in coordinating patient care, ensuring timely and effective treatment plans, and collaborating with an interdisciplinary team to optimize outcomes. This position involves travel to see patients, and mileage reimbursement is provided at the government rate.

Responsibilities
- Lead and coordinate patient care with multidisciplinary teams.
- Monitor care plans to ensure they align with patient needs and covered services.
- Collaborate with physicians on treatment plans, resource utilization, and discharge planning.
- Manage care for complex patients, including frequent emergency department visitors, chronic pain patients, and others.
- Oversee data collection for regulatory compliance and accreditation.
- Coordinate patient transfers to tertiary centers as needed.
Qualifications
Education and Experience:
- Bachelor’s Degree in Nursing (BSN) preferred.
- Minimum of 2 years of professional nursing experience required.
- Experience in Home Health preferred.
- Care management experience preferred.
Licenses/Certifications:
- Current Registered Nurse (RN) licensure in the state of California required.
- Case management certification preferred.
- Basic Life Support (BLS or equivalent) certification required.
Additional Requirements:
- Must have a clean driving record.
- Fully vaccinated (COVID-19, flu, etc.) or eligible for medical/religious exemptions.
