Children's Care Coordinator - Mercy Primary Care

We are looking for a Care Coordinator with strong skills to join our team. You will work with the children of families. You will join a team of Care Coordinators, family support persons, clinical leads, and medical/psychiatric/and nursing supports. We are looking for someone with the ability to manage a busy schedule, the needs of each family, and documentation needs.

Duties

• Participate in a team-based approach to care by collaborating with integrated disciplines located within Northern Light.
• Maintain records and documentation as required by Northern Light.
• Be familiar with specific outcome measures as required by Northern Light.
  
  

Qualifications

BA in related field required. Strong organization and communication skills. Need savvy computer skills to manage multiple systems. All applicants must maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving policy.
