SUMMARY : This is a part-time position to support medical records and case management services. This is a dual position that supports clinical operations. The position includes all medical transportation for patients and collaborates with the case managers to ensure patients have transportation to appointments. This position will also assist in fulfilling medical record requests.
RESPONSIBILITIES :
- Creating digital copies of medical paperwork and records and storing those records electronically
- Filing medical records accurately and in a timely manner
- Safeguarding medical records in accordance with HIPAA standards and regulations
- Ensuring that medical records are organized, accurate, and complete
- Learning and utilizing the EHR-integrated scanning software so providers can view scanned records within the patients’ EHR
- Fulfilling medical records requests
- Requesting medical records from outside providers as necessary
- Maintains a list of high-risk patients who require transportation to appointments
- Ensures patient signs consent forms and understands patient rights prior to transporting patients
- Collaborate with case managers to schedule appointments for patients and ensures availability for transportation

LANGUAGE SKILLS : Ability to read, analyze, and interpret common technical manuals and patient records. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the community. Ability to effectively present information to top management, co-workers, and public groups.
MATHEMATICAL SKILLS : Ability to understand with mathematical concepts such statistics. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
REASONING ABILITY : Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several concrete variables. Must have the ability to work with and relate to individuals from various socio-economic backgrounds. Must understand the ethical implications and patient confidentiality issues involved in this line of work, particularly in a small community. Must understand and accept their relationship and boundaries with patients and other staff members.
CERTIFICATES, LICENSES, REGISTRATIONS : Must successfully pass a background check. Requires BLS.

Requirements :
EDUCATION and / or EXPERIENCE : Associate’s Degree in a related field and 1 years of relevant experience, OR a High School Diploma / GED and 1 years of relevant experience and customer service.
