Title: Intake Coordinator
Reports to: Director of Nursing/Designee
Overview: The Intake Coordinator will be responsible for answering the direct referral line and processing in-coming referrals. This includes verifying third party payors and ensuring the referral meets regulatory guidelines and that the Agency can provide the appropriate services.
Responsibilities:
- Answer direct referral line within three rings.
- Ensure coverage for the direct referral line at all times.
- Maintain referral log.
- Complete referral/intake form on all in-coming referrals.
- Verify third party payor on all referrals and document on referral/intake form.
- Follows up accordingly on referrals and referral sources.
- Input data into the clinical system.
- Notifies the Director of Nursing of any problems/concerns and follows up on these problems/concerns according to the Director of Nursing input.
- Assists Staffing Coordinator to ensure referrals are being serviced appropriately.
- Processes in-coming reference requests.
- Assists in data inputting and transmission OASIS data within seven (7) days of the assessment completed.
- Assists in data inputting of 485 information timely and accurately.
- Assists in mailing out 485s and physician orders.
- Assists in answering main line phones when direct line not ringing.
- Demonstrating courteous, cooperative, and respectful behavior.
- Uses proper body mechanics
- Provides necessary information to maintain current employee file.
- Attends in-services/meetings as scheduled.
- Complies with polices and procedures.
- Practices safely and competently within job description.
- Communicates with supervisor about problems and other concerns.
- Utilizes good grooming habits and complies with dress code
- Performs other duties as assigned.
Qualifications:
- Licensed Registered Nurse in the State of which employed preferred. Degree - preferred.
- Minimum of two (2) years experience in healthcare home health related preferred.
- Computer literate.

Maintains compliance with Privacy Procedures Manual. Reports any non-compliance to Supervisor.
Access to active medical records.
No access to discharged medical records.
No access to employee files.
