Sonoma Specialty Hospital, as the county's only Long-Term Acute Care Hospital, is dedicated to caring for patients that require an extended acute care stay due to severe illness or catastrophic injury. We accept patients from the ICU, Step-Down and Telemetry units of partner hospitals and continue their care for as long as 45 days. This care focuses on the challenges of getting patients off of ventilator support, healing severe wounds, aiding their recovery following surgical procedures, and providing rehabilitation to speed their return to daily life. Our Hospital provides ICU, Surgical, and Step-Down care with dialysis, multi-specialty therapies and ongoing planning for the patient's transition back home or into a lower level of care. Job Summary :
Under the supervision of nursing manager, the Certified Nurse Assistant (CNA) assists Registered Nurses to provide quality nursing care to patients. Patient care duties may include traction set up, vital signs, reporting intake / output, weights, and interdepartmental transports.
Education and / or Experience
- CNA certification required.
- BLS certification required.
- One year's experience as CNA, Emergency Room Technician, or EMT in acute care preferred.
- Provides individualized attention, which encourages each patient's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
- Attending to the individual needs of patients. Examples include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the individuals' care requirements.
- Is knowledgeable of individualized care plans and provides support to patients according to their care plan.
- Maintains the comfort, privacy, and dignity of patients in the delivery of services to them. Interacts with patients in a manner that displays warmth and promotes a caring environment.
- Assists in maintaining a safe, neat, and clean environment; reports environmental deficiencies such as lighting or equipment problems.
- Performs various tasks assigned by the nurse manager, including checking vital signs, weighing patients, applying creams / ointments, and collecting specimens.
- Lifts, moves, and transports patients, using proper body mechanics or lifting devices for accident prevention.
- Practices careful and efficient use of supplies and linen and follows established charge procedures for patient charge items.
- Completes certified nursing assistant records documenting care provided or other information in keeping with department policies.
- Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves were indicated and proper disposal of soiled materials.
- Has initiative in organizing work and completing assignments.
- Cooperates with staff to maintain a well-organized department.
- Always demonstrates professional decorum.
- Is willing to accept direction from the team leader.
- Maintains tactful and cheerful relationships with patients and visitors. Must use tact in dealing with relatives and friends of patients, including respecting patient confidentiality.
- Must read, speak, write, and comprehend English.
- Computer skills required. Basic math knowledge, including addition, subtraction, and multiplication.
