SSM Health Oklahoma is seeking a dedicated and compassionate physician to join our team. The clinic operates as a family medicine clinic but does have a small population that focuses primarily on providing medical care to older adults in nursing homes and skilled nursing facilities.

Key Responsibilities
- Patient Care:
- Provide comprehensive medical care to residents in nursing homes and skilled nursing facilities.
- Conduct regular patient visits, assessments, and follow-ups to monitor and manage chronic conditions, acute illnesses, and overall health.
- Develop and implement individualized care plans in collaboration with nursing staff, therapists, and other healthcare professionals.
- Ability to make rounds in the nursing home facilities and see hospice patients as well.
- Coordination and Communication:
- Coordinate care with other healthcare providers, including specialists, hospitalists, and primary care physicians, to ensure continuity of care.
- Communicate effectively with patients, families, and caregivers to provide education, support, and guidance on health-related issues
- 1.0 FTE; Monday - Friday
- (4 1/2 days of patient-facing days with 1/2 day of administrative)
- Block patient times start at 20/40 but will shift to 15/30
- Building your own panel while absorbing some for a retiring physician.
- In-office procedures:
- skin tag removals, foreign body removal, Lapalme execution, sebaceous cyst, joint injections, lacerations
- Call:
- Weekday call starts at 5p and goes to 7a
- Weekend calls is both days receiving calls from home health nurses and hospice nurses from local institutions
- The clinic can accommodate 4 providers comfortably
- Longevity: two providers have been with SSM for 25 years; the other 11 years
- Large SSM referral network
- You will have your own office
- Lab on site with point of care.
- Medical degree (MD or DO) with board certification in geriatrics, internal medicine, or family medicine.
- Valid medical license to practice in [Your State].
- Experience working with older adult populations in nursing homes or skilled nursing facilities is preferred.
- Strong clinical skills and knowledge of geriatric syndromes, chronic disease management, and palliative care.
- Excellent communication, interpersonal, and organizational skills.
- Compassionate and patient-centered approach to care.
- The clinic functions as a family medicine clinic
