Responsibilities:
- Assess patient needs and develop comprehensive care plans in collaboration with interdisciplinary teams
- Provide skilled nursing services, including wound care, IV administration, and medication management
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
- Document visit notes, treatments, and care in accordance with regulations and policies
Requirements:
- Current RN licensure in the state of employment
- Basic Life Support (BLS) certification
- Valid driver's license and reliable transportation
- Ability to lift up to 25 pounds and navigate various environments
