Responsibilities
We are seeking an experienced nursing professional to join our team as a Referral Liaison. The successful candidate will be responsible for:
- Creating a positive patient care experience by greeting, preparing, exiting, and following up with patients for providers according to established competency guidelines, policies, and procedures.
- Providing support, both expected and directed, and collaborating with physicians and multi-disciplinary care teams for best care/results.
- Ensuring accuracy of data collection and reporting for quality initiatives.
- Applying existing evidence-based practice and scientific knowledge in healthcare to nursing practice, ensuring that nursing care is delivered based on patient's age-specific needs and clinical needs as described in the department's Scope of Service.
