Patient Advocate and Care Manager

Job Description:

This RN position is responsible for assessing, developing, implementing, and evaluating discharge planning needs for patients. The successful candidate will collaborate with interdisciplinary teams to ensure seamless transitions between levels of care.

The RN will also be responsible for:

• Ensuring compliance with Condition of Participation (CoP) discharge planning.
• Assessing patients at high risk for readmission and developing plans to prevent such events.
• Collaborating with multidisciplinary teams regarding discharge planning to next level of care.
