Job Overview:
- This is a full-time position working as a RN Home Health Care Provider, traveling to patient homes, Community-Based Residential Facilities (CBRF), assisted living locations, hospitals, or other settings to meet patient and family/caregivers' needs.
- You will coordinate and communicate with multi-disciplinary team members and outside agencies as appropriate to achieve expected outcomes for patient care and education.
Responsibilities:
- Assess and coordinates patient's discharge planning needs with members of the healthcare team
- Perform point of care testing according to policies and procedures
- Collaborate, consult, and communicates with physician regarding the plan of care and changes to patient status
Requirements:
- Basic Life Support HealthCare Provider (BLS HCP) certification
- Registered Professional Nurse (RN) licensure in Illinois, Missouri, Oklahoma, or Wisconsin
- Valid driver's license
Work Environment:
This role requires frequent driving and occasional lifting/moving of patients.
