Job Description
The Skilled Nursing Professional - Home Setting will be responsible for assessing and coordinating patient's discharge planning needs with members of the healthcare team. This includes performing point-of-care testing, collaborating with physicians, and communicating with multi-disciplinary team members and outside agencies.
Responsibilities:
- Assess patient's medical condition and develop a plan of care.
- Collaborate with interdisciplinary team members to achieve expected outcomes for patient care and education.
- Communicate regularly with patients, families, and healthcare teams to ensure seamless care transitions.
- Document visit notes, treatments, and care provided in accordance with regulatory requirements.
Requirements:
- Registered Professional Nurse (RN) license in good standing.
- Basic Life Support (BLS) certification.
