The Role
As a Care Coordination RN, you will play a vital role in ensuring that patients receive seamless, coordinated care. You will assess patient care needs, develop individualized plans of care, and collaborate with multidisciplinary teams to achieve optimal patient outcomes.
Duties and Responsibilities
- Coordinate patient's discharge planning needs with members of the healthcare team.
- Develop comprehensive plans of care based on unique patient needs, incorporating input from healthcare team members and patient/family preferences.
Requirements
- Graduate of accredited school of nursing or education equivalency for licensing.
- Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA).
- Registered Nurse (RN) issued by compact state or Missouri Division of Professional Registration, Oklahoma Board of Nursing (OBN), or Wisconsin Department of Safety and Professional Services.
