Job Summary
We are seeking a compassionate Home Health Nurse to join our team in Reedsburg, WI. The ideal candidate will be responsible for coordinating and facilitating patient care through assessment and evaluation of patient condition.
Responsibilities:
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Performs point of care testing according to policies and procedures.
- Collaborates with physician regarding the plan of care and changes to patient status.
- Provides skilled nursing care within the established plan of care.
- Documents visit notes, treatments, and care.
Requirements:
- Graduate of accredited school of nursing or education equivalency for licensing.
- No experience required.
Work Environment:
The successful candidate will work primarily in patient homes, Community-Based Residential Facilities (CBRF), assisted living locations, hospitals, or other settings as needed. They will also be expected to drive regularly and use their vehicle for business purposes.
