The Case Manager is responsible for clinical and medical case management for all Lifeline clients.

Duties & Responsibilities
- One on one case management for person with substance abuse disorders.
- Active participation as a member of the multi-disciplinary team that develops and supports the client’s service plan.
- Coordinate care/services with SSTAR open access model and primary care needs.
- Awareness of community resources and referral mechanisms.
- Identifies gaps in service and advocates appropriately.
- Maintain confidentiality to the fullest extent.
- Assist with transportation arrangements.
- Represent program/agency internally and externally as requested.
- Accepts and participates in appropriate supervision.
- Other duties as required.
Experience and Skills:
Education & Experience
- Bachelor’s degree in social work or related field or CADC.
- Strong verbal and written communication skills.
- Open to working with people from diverse backgrounds, cultures, and lifestyles in a non-judgmental manner.
