Job Status: Full-Time, Days

Position Summary:
1. Performs the daily medical and nursing functions in accordance with current rules, regulations and guidelines that govern St. Ann Center for Intergenerational Care and the Personal Hygiene Program.
2. Reviews client I.S.P. for appropriate care based on individual nursing needs and administer professional nursing services as needed and/or as ordered by the physician.
3. Consults with the client's physician and caregiver, in close collaboration with the Director concerning treatment or when there is a change in client's condition.
4. Performs administrative duties, which include completing medical forms, reports, evaluations and charting.
5. Assures that St. Ann Center for Intergenerational Care personnel, clients and visitors follow established policies and procedures at all times, relative to nursing and medical needs.
6. Implements an effective orientation for new employees and continue education for current adult day care staff in collaboration with Registered Nurse.
7. Contacts family/caregiver to refill prescribed medications, supplies, and equipment.
8. Meets with clients and family members as necessary for any follow-up in-home care, in close collaboration with the Director.
9. Assists with any client care as needed.
10. Attends weekly staff meetings and quarterly all-staff meetings.
11. Coordinates doctor’s orders with doctor's staff, home health agencies, and caregivers, and other health care facilities for continuity of care.
12. Attends and participates in professional organizations’ in services and meetings.
13. Participates in weekly multidisciplinary staffing.
14. First responsible to all emergencies with appropriate follow up for all staff, child, adults, volunteers and guests at the Weinberg Building.
Qualifications
Education/Training:
Licensed LPN – State of Wisconsin
Some previous experience as a Licensed Practical Nurse preferred
Must follow all Policies and Procedures of St. Ann Center for Intergenerational Care

Salary : $20 - $22
