Personal Care
- PA Nursing License LPN or RN and be in Good Standing

Job Description:
The Resident Care Coordinator (LPN) Personal Care is responsible for overseeing the care of residents in our personal care department (Villa Angela). The coordinator will be responsible for training and supervising staff, interacting with residents, and ensuring that all care is provided with compassion and empathy. The coordinator will organize, and problem solve any issues that arise, and will communicate with other health team members to ensure that all residents receive the best possible care. The coordinator will also be responsible for documenting all care provided, consulting with doctors as needed, and interacting with family members to ensure that they are informed and involved in the care of their loved ones.
- Interaction with residents and have access to the residents property.
- Collect data to help develop RASP
- Train, Supervise, Motivate, and Support staff-ensuring highest of quality of care
- Be a team player
- Perform other duties as assigned
- Provide case management services to residents'
- Encourage family involvement with residents
- Monitor and maintain records reflecting physical and mental conditions of residents
- Reporting changes in conditions
- MAINTAINING CONFIDENTIALITY
- Perform other duties as assigned
- Thorough and accurate nursing reports
- Communicates with physicians, hospitals, hospice, home health
- Accurate documentation and record keeping
