Job Description

I.          TITLE:                                  Therapist – Community Based Clinical and Care    

                                                             Management Services

II.        SUPERVISOR:                     Program Administrator - Community Based Clinical and

                                                             Care Management Services - Hudson Falls

III.       HOURS PER WEEK:          40 hours (includes some early evening hours)

IV.       QUALIFICATIONS:           LMSW/LCSW, LMHC or other relevant degree and

experience working with individuals, families and groups preferred. Those with MSW may be considered with the expectation of acquiring licensure ASAP.  Ability to function as part of a multi-disciplinary treatment team. Excellent communication and written skills required.

V.        FUNCTIONS:                       To provide individual, family therapy and group therapy to

assigned clients. 

We are looking for a dynamic, trauma informed candidate to be part of our clinical team. In this role you will work to help children and families, in our sexual abuse prevention programs as well as justice-involved individuals, with the goal of preventing future harm and fostering healing.   Experience with electronic health records, CFTSS (Children and Families Support Services) and NYS OCFS CONNECTIONS, and credentialed for insurances is preferred. Position is located in our Hudson Falls office, serving Saratoga, Warren, and Washington counties.

VI.       RESPONSIBILITIES:         

• Coordinate the clinical treatment plan and develop comprehensive clinical assessments and treatment plans for assigned individuals and families.
• Provide individual, group and family focused treatment/therapy including outreach and advocacy.
• Serve as the ongoing liaison with community agencies and referral sources.
• Coordinate and complete necessary reports and correspondence
• Attend weekly individual and team/group supervision.
• Facilitate case planning for each client/family.
• Work cooperatively with client’s case manager and/or outside service providers to coordinate clinical services and resolve issues. 
• Maintain updated assessments and progress notes, and record updates in the NYS OCFS CONNECTIONS electronic case management system and/or in the SAI Electronic Health Record as required by contract or regulation.
• Present clinical treatment summaries on clients at weekly clinical team meetings.
• Attend all clinical department staff meetings and supervisory meetings, as required.
• Keep abreast of current trends in clinical treatment, trauma-informed care and evidence-based models through reading, in-service training and conferences.
• Adhere to NASW, AMHCA, NYS Justice Center and SAI Codes of Conduct/Ethics, and ATSA Guidelines.
