PRINCIPAL RESPONSIBILITIES :
Under the direct supervision of the Clinic Director of St. Anthony Medical Clinic (SAMC), the Licensed Clinical Therapist / Social Worker will provide therapeutic mental health services to clients of the SAMC and other St. Anthony Foundation programs. The provider will function as part of the primary care team at the Clinic. The individual will have demonstrated skills and ability in promoting excellent patient care in a patient-centered medical home setting with integrated primary care and behavioral health, and will also act as a behavioral health resource and consultant to the health care team that includes medical providers, support staff, health educators and other St. Anthony programs.
MAJOR DUTIES :
- Provide change-focused short term therapy, consultations and referrals for Clinic patients on a drop-in, appointed time and group visit basis.
- Provide counseling services for individuals, families, and groups, as appropriate.
- Conduct clinical assessments; develop treatment plans, and appropriate services and follow-up.
- Refer clients for behavioral health and other services as appropriate.
- Exchange information (within legal parameters) with other health and / or rehabilitative agencies to provide continuity of care for clients receiving services from more than one agency.
- Facilitate case review meetings and participate in staff meetings, agency meetings and other meetings as assigned.
- Serve as a crisis counselor for patients.
- Work in an integrated setting offering collaborative care with primary care teams via consultation, shared patient visits, warm hand-offs or other forms of patient care integration activities.
- Serve on committees and participate in various health center projects as assigned.
- Assist in the design and implementation of behavioral health and collaborative care protocols.
- Licensed LCSW or Licensed Psychologist to provide workflow review and training of the Behavior Heath content in the Electronic Health Records (EHR).
- Dual signature on all charts; Periodic review and chart audit including Peer Review to ensure compliance of charting protocol.
- Complete all necessary reporting of supervision hours to the corresponding board or licensure agency of the supervisee.
- Perform other duties as assigned.
