Responsibilities

1. Nursing Assessments: Perform comprehensive nursing assessments to identify patient needs and develop individualized plans of care.
2. Collaboration: Work closely with the nursing team, other disciplines, patients, and families to ensure effective care delivery.
3. Goal Setting: Establish measurable short- and long-term patient goals, with interventions in place to minimize rehospitalizations.
4. Communication: Communicate patient information effectively with assigned staff and between shifts.
