CommonSpirit St. Anthony North Hospital (SAN) is a not-for-profit, faith-based hospital in Westminster, Colorado. SAN operates the 84th Avenue Freestanding Emergency Department in Federal Heights, Colorado at the site of the legacy St. Anthony North Hospital’s previous location. SAN primarily serves populations in Adams and Jefferson Counties. The catchment area includes the cities of Westminster, Erie, Brighton, Broomfield, Northglenn, Thornton, and Fort Lupton. SAN has ~121 licensed beds and operates as a community hospital with comprehensive services including trauma & acute care surgery, bariatric surgery, medical sub-specialty care, comprehensive obstetrical care, gynecologic care, primary care, orthopedics including joint replacement, outpatient spine surgery, and comprehensive emergency care. The hospital serves as a Level 3 trauma center, an accredited stroke center and a primary PCI center for its catchment area. The combined emergency departments at SAN and the 84th Avenue FSED see approximately 55,000 patient visits per year.
Overview:
We are looking for a Patient Navigator to join us at St Anthony North:
Job Summary:
- Support routine opt-out testing and treatment for STIs, HIV, and HCV in the emergency department.
- Advocate for and normalize routine opt-out testing in the ED, including posting signage and outreach materials.
- Develop connections with community providers to facilitate patient referrals.
- Make Navigator contact information widely available and respond to inquiries from ED and community providers.
- Educate patients about the effects, treatment options, and chronic disease management of syphilis, HIV, and HCV.
- Coach patients in preventive practices and harm-reduction strategies using motivational interviewing techniques.
- Work with Clinical Champion and Program Coordinator to set up a follow-up system for patient notification and treatment.
- Facilitate treatment of syphliis, HIV, and/or HCV via the emergency department or community clinics/agencies
- Enter encounter data into electronic health records and other data collection systems.
- Collect and report monthly ED metrics as required by funders.
Qualifications:
Education Requirements:

Bachelor Degree Required Bachelors degree in Nursing (Bachelors degree
in Public Health, Health Sciences or closely related field may substitute)
Minimum Experience Requirements:
- Two years of experience providing community health education services (academic degrees in Public Health, Health Sciences, or a closely related field may substitute for experience on a year-for-year basis)
- Bilingual skills (Spanish) preferred
- Ability to work independently and as part of a team, be flexible, and manage multiple assignments in a dynamic, fast-paced environment.
- Ability to work effectively with varied populations, including patients with substance use disorders, mental health diagnoses, and high risk sexual behaviors
- Strong understanding of HIV/STD-related issues and working knowledge of Medicaid and other public programs.
