Vision: St. Augustine Society Inc. envisions a future in which all those in St. Johns County have a safe, permanent place to call home.The Case Manager works collaboratively as part of the Operations Team to support the mission of St. Augustine Society, Inc. by being responsible for the case management of clients in the Housing Crisis and Advocacy and Referral Center Programs. The Case Manager assist clients with self-sufficiency by specializing in housing-focused Individual Service Plans, working with clients to obtain the housing option most appropriate for, and identified by the client along with client-identified supports to ensure ongoing housing stability. This position requires a clear communicator-both written and verbal, organized with effective time management skills, demonstrating a willingness to learn and adapt, with the highest standards of behavior, collaborative manner and work ethic. Successful candidates must have the ability to cover various shifts including weekends and holidays. This position reports to the Program Manager. Specific duties include assisting clients in achieving self-sufficiency through needs assessment, goal identification, individual service planning, and resource referrals, developing and maintaining appropriate records and documentation of services to clients. The Case Manager will coordinate the efforts of the Direct Care Staff in the forward movement of all clients.

Sunday -Wednesday 8am to 6pm 10 hour shift 4days on 3 days off

Qualifications, Experience and Abilities:

· As a prerequisite, the successful candidate must believe in the core values and philosophy of St. Augustine Society, Inc. and be driven by the mission of the organization with a commitment to working with persons in housing crisis and disconnected populations

· Bachelor’s degree in Human Services related field preferred

· Two years’ experience working in Human Services working with one or more of the following: individuals experiencing homelessness, mental health related issues, substance abuse, domestic violence, and with those living in poverty

· Ability to establish and maintain professional boundaries in working with clients

· Knowledge of community resources in the St. Johns County area preferred

· Experience working with clients with multiple barriers to income and housing stability preferred

· Ability to manage multiple projects with demanding and competing deadlines, superior organizational skills and the ability to maintain a quality workplace in a diverse, fast paced, stressful and changing environment

· Ability to work independently as necessary. Able to work with minimum day to day supervision

· Demonstrates professional development by participating in and seeking out higher-level, innovative training opportunities

· Ability and willingness to perform light housekeeping and occasional lifting to 30 pounds independently

· Demonstrates a positive attitude, self-motivation, organization, and resourcefulness while maintaining a reputation through proven action of being team and client oriented and willing to assist where/when needed

· Ability to negotiate different personalities and work under competing priorities

· Ability to assess emergency situations and respond effectively

· Excellent written and verbal skills

· Demonstrated proficiency in using Microsoft Outlook, Word, Excel required. Experience with other applications as well as experience using and entering data into HMIS or other client databases preferred

· Must pass Level II background check

· Must pass drug screen

· Must be prompt and dependable

Essential Duties and Responsibilities

· Determines clients’ requirements by completing interviews; determining need for service connection and support to successfully access housing alternative to shelter

· Develops clients’ Individual Service Plan using client-centered approach

· Monitors progress on ISP through routine, regular meetings with clients

· Maintains clients’ records by reviewing case notes; logging events and progress

· Delivers services with safety, respect and effectiveness

· Establishes and maintains effective relationships with outside agencies and resources

· Works with the Program Manager to ensure service delivery is consistent with current evidenced-based and best-practice models

· Assists the Program Manager in ensuring accurate data is collected to support grant applications

· Participates in rotating on-call consultation duties

· Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks

· Assists Direct Care Staff to monitor and supervise clients and milieu, meet client needs and questions and maintain shelter order in accordance with policy as needed

· Assists Direct Care Staff to receive new clients, complete intake paperwork, create proper client files and orient new clients to agency services and policies as needed

· Enforces agency policies and follows staff policies and procedures

· Assists Direct Care Staff to perform general administrative functions such as noting within the communication log, answering the phone and responding to in-person inquiries in a professional manner, and providing appropriate information and referrals for those who need other resources as needed

· Maintains order and assists in keeping all assigned areas clean and free of spills, trash and safety hazards

· Provides immediate and professional response to any security/safety emergency or disturbance within or immediately outside facility

· Maintains written and oral communication of incidents in accordance with policies

· Reports any problems to the Program Manager

· Attends staff meetings and participates in activities designated by the Program Manager/Operations Manager/Executive Director

· Performs all other duties as assigned

General Job Performance Evaluation Standards

· Understands and follow all agency policies and procedures as directed by personnel manual and Program Manager’s guidance

· Performs all duties listed in Job Description

· Maintains strict client and staff confidentiality at all times

· Represents the agency in a professional manner consistent with the St. Francis House mission

· Attends meetings as required including all mandatory staff meetings, any necessary emergency meetings, and staff training. Participate in required and elective staff development activities

· Provides supervision and support to clients

· Maintains a safe environment for all clients

· Completes assigned paperwork
